Die-off (sometimes called a Herxheimer reaction or Herxing) is a temporary increase in symptoms some people feel when microbes are being killed during SIBO (bacterial overgrowth) or SIFO (yeast overgrowth) treatment. Classically, it starts on day 2 and lasts about 2 days, but it can start right away or later, and can last longer. It can be helped by various treatments or by lowering treatment dose. What is Die-Off? Die-off refers to symptoms that can occur when microbes die and release substances that can trigger inflammation or detox-like symptoms. It's caused by the immune system's reaction, including the release of inflammatory cytokines, to what it thinks is infection, but is actually increased pieces of bacterial cell wall (called LPS or endotoxin) or other released microbial components and toxins, as they break apart and die with SIBO treatment. In clinical experience, die-off tends to be more intense with yeast, than with bacteria, which is why it comes up in discussions of SIFO (yeast/candida/fungal overgrowth) as well as SIBO. Very bad or long die-off when using herbal antibiotics for SIBO, can indicate yeast overgrowth. Die-off can occur in all overgrowth patterns, including SIBO, IMO (methane SIBO), ISO (hydrogen sulfide SIBO) and SIFO. See more about SIBO types here. Die-Off Symptoms Symptoms can vary individually and in intensity, from mild to awful. When die-off is moderate to severe, it can interfere with one's ability to work and function normally (as if one had the flu). Clinically, the most common die-off symptoms I observe during SIBO treatment are bloating, fatigue/malaise and headache.

Does Everyone Get Die-Off? No, many people don't get die off at all, during any type of SIBO (or SIFO) treatment, even if they have high gas. It can be very hard for even experienced clinicians to tell the difference, because die-off symptoms can overlap with side effects or an individuals reaction to a medicine (i.e. gastrointestinal symptoms could be from any of the above). Symptom aggravation that resolves during the treatment, as well as normal liver tests, point to die-off. [2] However, die-off can last an entire treatment. What are SIBO die-off symptoms?+
SIBO die-off symptoms include fatigue, malaise, headaches, flu-like feeling (including body or joint aches, upper respiratory symptoms), brain fog, worsening of current digestive symptoms (especially bloating) or new digestive symptoms.
How long does SIBO die-off last?+
Classically, onset is on day 2–4 and lasts 1–3 days. However, it can start on day 1, or on week two or three of longer treatments, and can last the duration of the treatment (4-6 weeks for herbal antibiotics). Prolonged or intense die-off is more common with high gas levels (≥ 70 ppm) or when yeast overgrowth is also present. Intense die-off when using herbal antibiotics, raises suspicion of yeast overgrowth.
What helps die-off during SIBO treatment?+
Die-off treatments include anti-inflammatories such as ibuprofen, turmeric, or vitamin C (see above for more), activated charcoal, IgG or colostrum, and alkalinizing agents (e.g., Alka-Seltzer), as well as Epsom salt baths or sauna. Another common strategy is to lower the treatment dose by half until symptoms clear, then slowly increase back up — consult a clinician before changing any prescribed dose.
Is die-off the same as a Herxheimer reaction?+
Yes. “Herxheimer,” “Herx reaction,” or “Herxing” are synonymous with die-off. These terms are more frequently used in discussions of Lyme disease, while “die-off” is more common in gut overgrowth discussions.
Why do I feel worse after starting SIBO treatment?+
It could be die-off, which can cause a temporary increase in symptoms. Other possibilities include medication side effects (for example, nausea from antibiotics) or an individual medicine intolerance or allergy (a symptom reaction to a specific ingredient that is unique to you).

This video was so helpful! I was doing a healthy plant-based cleanse, low histamine and fodmap diets all at once. I was pretty sure I had covid or the flu.It was really bad. Nothing worked and I had to reintroduce some of the "forbidden" foods to feel better... But the SIBO or SIFO are still very much with me.
